The Limits of Anomaly Detection in Healthcare
For hospital security operations centers (SOCs), seeing an anomaly is only the beginning. A 3 a.m. login with valid credentials might look like a breach, but in a healthcare setting, it could be a traveling radiologist accessing patient records or a night shift nurse reporting early. Without real time context from HR systems and identity and access management (IAM) platforms, security teams cannot distinguish between a legitimate action and a threat. As Sujatha S Iyer, head of AI security at ManageEngine, explains, “You need different tools bringing in the context so that you have a decision that is much more accountable, explainable and context aware.” For healthcare organizations, where a false positive can delay critical clinical workflows, this capability is essential for both security and patient safety.
Dangers of Overly Permissive AI Access
The same context gap appears in agentic artificial intelligence (AI) deployments now entering healthcare environments. When health systems deploy AI agents for tasks like scheduling or clinical decision support, they often grant admin level access for convenience, leaving a single exposed API key as the only barrier to sensitive patient data (ePHI). Iyer recommends scoping agent access strictly to required functions and securing the data access layer from the start. For a hospital CISO, this means ensuring that AI agents cannot reach beyond their designated datasets, such as preventing a scheduling AI from querying lab results. This aligns with HIPAA’s minimum necessary standard and reduces the blast radius of a compromised agent.
What This Means for Healthcare Organizations
For healthcare security leaders, the takeaway is clear: context aware decisions are not optional in a field where patient data and clinical uptime are at stake. Integrating HR systems, IAM tools, and AI security controls into a unified view helps the SOC triage alerts without disrupting care. When an AI agent’s action looks suspicious, that same integration can cross reference it against employee schedules or device location logs. By baking security into the design of AI systems—not bolting it on afterward—health systems can comply with FDA premarket guidance for AI enabled medical devices and avoid costly breaches. As Iyer notes, the goal is “a decision that is much more accountable, explainable and context aware.” For the average hospital, this means investing in tools that bridge data silos and training staff to question what lies beneath every alert.
